FEES

CONSULTATION FEES:

Patients are strongly recommended to have Japanese health insurance
in order to keep consultation fees at a minimum.  Fees are normally higher
for the initial visit depending on whether medical tests are undertaken or
not.  Fees for subsequent visits are kept to a basic rate.

PATIENTS WITH HEALTH INSURANCE:

First visit without any medical tests:      3000 yen (approx)

First visit with medical tests:                  6000 – 8000 yen (approx)

Subsequent visits:                                   1500 – 1740 yen (approx)

PATIENTS WITHOUT HEALTH INSURANCE:

First visit without any medical tests:      9300 yen (approx)

First visit with medical tests:                  24000 yen (approx)

Subsequent visits:                                   5000 yen (approx)

MEDICAL CERTIFICATE FEES:

Patients who are in need of a medical certificate will be gladly provided with
the required documentation.  The following fees apply:

Certificate - clinic original format:   3000 yen

Certificate - clinic original format (with translation):   5000 yen

Certificate - patient's requested format:  4000 yen

Certificate -patient's requested format (with translation): 6000 yen
